Each process is described in a step-by-step fashion.

QA / Compliance Recordkeeping Binder System

The QA/Compliance Recordkeeping Binder System is a detailed, comprehensive filing system for maintaining compliance with state/federal radiation safety regulations and accreditation standards. Designed for easy filing, the binder system organizes documents into QA, radiation safety, and quality management program areas. Depending on the center’s capabilities, the binder system is comprised of up to twenty-nine (29) binders (Avery heavy-duty, white 3-ring binders) and 139 dividers with tabs. Twenty-two (22) of the 29 binders are designed to be maintained in a central location (e.g., such as dosimetry). These binder are entitled: Training, Communications, Facility Specific Procedures, Regulations, State License/Registration, Accelerator Data, CT Simulator Data, Treatment Planning System, Record and Verify (EMR) System, Clinical QA-Physics, Clinical QA-Therapist, Clinical QA-Physician, Forms, Instrumentation, Accelerator QA-Physics, CT Simulator QA-Physics, Film Processor QA (if applicable), Block Cutting QA (if applicable), Audits and Inspections, Incidents, Committees, and Personnel Dosimetry. Error Reduction Training

Error reduction training is available at your facility as a 2-day course. The Methodology and Patient Safety Course is offered as advanced training in the MERP software application. This course provides in-depth training in MERP’s comprehensive and systematic approach for reducing risk in radiation oncology. NOTE: Initial, basic training is provided as part of the MERP installation. The prerequisite for advanced training is the initial MERP software application training.

History: Start by reviewing the history of medical error reduction and risk management in medicine. Look at current and future mandates by the states and federal government to reduce medical errors. Examine error rates and adverse events in radiation oncology.

Process: The class will reinforce the theory and functionality behind the MERP software application. Certain administrative features for customization will be presented.

  • Learn to enter, evaluate, correct, and trend real-world scenarios of errors found in radiation oncology.
  • Understand how to capture “Near Misses” and recognize opportunities for improvement.
  • Examine contributing factors and perform root cause analysis.
  • Walk thru completing sentinel and medical event reports.
  • Learn to develop effective action plans to prevent recurrence of problems.
  • Integrate facility or RPS programs/procedures (if purchased) as part of fix.
  • Bring actual clinical problems to the course and see how MERP works “real-time” by solving the event.

Custom Individual Error Feedback

Individual error feedback can be incorporated as part of the MERP software or paper-based error reduction programs. RadPhysics provides independent, third party review of all errors in a secure and confidential fashion. The MERP application can be configured to include RadPhysics review using a remote access data link (e.g., internet, VPN). The paper-based process can incorporate RadPhysics review via facsimile. In both cases, RadPhysics provides timely feedback in processing of each error. Each error is reviewed for accuracy, credibility, and appropriateness:

  • Proper selection of error type
  • Correct assignment of error category, subcategory, and attribute
  • Valid assessment of clinical and regulatory significance
  • Verifiable results of dose error calculation wizard
  • Suitable short and long-term corrective actions
  • Effective drill-down in root cause analysis

Standards / Regulations Updates

MERP

Keeping current with ACR, ACRO, and The Joint Commission standards as well as state radiation control agency regulations can be very demanding and time consuming. RadPhysics does the footwork for you. RadPhysics tracks changes in these professional standards and your applicable state’s radiation control agency regulations. As updates are released, RadPhysics monitors, reviews, and applies the changes to the MERP software application. The changes are reflected in MERP reference features such as error attributes, information buttons, regulatory infractions, sentinel events, root cause analysis, and medical events.
